The Undead Skeleton is a Dungeon Mob that spawns in The Catacombs and below.

Spawning

Undead Skeletons spawn when 3 Skeleton Skulls, which are spawned when a Skeleton-type Mob is killed, are close to each other. They will group together and form into an Undead Skeleton. This can happen on any floor in The Catacombs.
